Hi guys!

For context I have the following conditions: heds (8/9 beighton), POTS (confirmed via tilt table), had hyoid bone syndrome in my neck (had surgery - they trimmed both sides. I would say it helped - especially my neck now feels more “symmetric” swallow feels smoother, no more TIA mini stroke symptoms) but still have some neck problems I will detail later below, cervical instability (neck vertebrae always clunking), c5 c6 degenerative autofusion (non klippel feil - they just fused due to instability I guess, RCPD (lack of ability to burp - I tried the botox shot no effect didn’t really do much for me and still cannot burp), occult tethered cord syndrome surgical release (significantly helped me, had this a few months back so continually rehabbing my left leg - the affected leg but am way less constipated after walking, TMJ of course, and potential thoracic outlet syndrome on left side.

Now for what my main complaints are: Essentially every time I eat, I get strange symptoms. Primarily left sided around neck. I can’t really tell what is going on. It feels vascular to me. Maintaining good posture like forcing it seems to not help (hurts actually). what helps is to lean my head back onto the head rest and relax my jaw, particularly on the left side. Also lowering my left shoulder then slowly bending my neck to the right I get a pop somewhere in the left side of the neck that helps symptoms. I am a bit confused if the issue is thoracic outlet or more eagle at this point. Sometimes I feel like my left ear doesn’t really work as well as the right, and little sounds in that ear. My main complaints though are this weird left side of neck “tingling” which I can’t really tell if it is vascular or more “nerve” and insane head pressure at the front of my skull. Looking at MRI I don’t think I have chiari, never been mentioned and I have had every scan under the rainbow. Symptoms seem to improve when I go on my bike for a few minutes or I walk around to get blood flowing, but this doesn’t really seem like the presentation of pots that people complain about. I have dramatically improved my health the last 2 years, and I would say my legs and core are pretty strong… while it helps me feel better in general the left neck thing is still there. I think chewing particularly chewy things on the left side of the jaw seem to worsen things, so I prefer lots of softer foods.

So… looking at my CTA images. My hyoid was gigantic, touching carotids… got it corrected, shouldn’t be the issue anymore. When I ran the DICOMS through a 3D slicer image combiner program, I could see that the left styloid was segmented (both styloids are not elongated though). What I suspect, but I have no real proof is that a positional symptom is occurring when I eat for that small segment of the styloid on the left side. I would be really happy to share images with anyone for them to look at. I am getting a second opinion from my hyoid bone syndrome doc, as he was initially dismissive of my eagle concerns considering my non elongated styloids.

Is what I suspect plausible? How would I even prove this to the new doc… Would anyone be willing to take a look at my images? Sorry for the long post, and thank you guys it is great to join this community.

I should add for the hyoid bone syndrome surgery I had the following done as well: stylohyoid ligament release, as well as suturing of the hyoid further up in order to improve the swallowing (doc recommended and I approved).

I also think that swallowing/dysphagia problems may be due to my hyoid bone. It is also possible that when it rises, it innervates the nerves or even the nerves of the carotid artery. I also want to discuss this with the surgeon, from both sides. I figured out why my hyoid bone is so wide, and perhaps for the same reason the digastric muscle is hypertrophied (inflated). Digastric muscle hypertrophy is associated with two variants: 1) the main assumption is that the horns of the hyoid bone enter the long neck muscle (which is attached to the skull) . I.e. her large horns slip or get stuck in the tendons of a long muscle (this explains possible nerve innervation and swallowing problems/dysphagia). Since the horns slide along the expanding muscle, or are stuck in individual fibers of the biceps muscle, you have to strain harder, and perhaps the horns of the hyoid bone are lower than they should be and a constant tension of the muscle is created. This is also the reason why calcification of the stylo-hyoid ligament occurred (when it was still working).
2) The digastric muscle runs diagonally through the transverse process of C1, thereby rubbing against C1 (perhaps this is why it is hypertrophied, but this is very unlikely, it must be stretched. And it is rather stretched for the 1st reason).
It is not clear here, the large and small horns were formed as protection and for a fulcrum to lower the hyoid bone lower and thus the digastric lower, perhaps it used to be higher, Or maybe it’s just a bone anomaly. But there is a high probability of problems with swallowing due to the hyoid bone. That is, something needs to be solved with the bone.

Tell me in more detail what was done to your hyoid bone. Were the greater horns of the hyoid bone resected, with their edges near the common carotid artery? Or farther away from the common carotid artery? Was the incision made near the carotid artery, or was the hyoid bone pulled back and moved to make the incision farther away from the carotid artery? Or was something else done?

I understood what was sutured? Tell me more. Did they reposition your hyoid bone, i.e., move it closer to the chin (using special tools and screws in the chin bone)? And did they partially resect the edges of the greater horns of the hyoid bone, or not?

Yes one side was significantly more calcified and longer (right side) but the doc said if he was going in he was going to cut both sides, so he cut both sides (greater horns). Yes the right edge was directly touching carotid, my worst complaint was when i put my head down and to the right I felt my left side go numb a bit and my brain felt very funny. The incision was horizontal across the length of the hyoid boneto access both sides (literally looks like someone took a knife and slit my throat horizontally). Your last series of questions - I am not sure. I told the doc I have always suffered severe swallowing difficulties. So he suggested to suture the hyoid higher up to a group of muscles I believe (I asked if he was suturing to the thyroid cartilage and he said NO - not to this structure). There are no screws or hardware involved - my understanding is only the sutures that degrade over time were used. I have noticed that after this procedure, I am able to raise and lower my hyoid at will by suctioning the top of my mouth (was not able to do this before).

You should be able to upload images now if you want to? Obviously we’re not doctors here, but might be able to give you some thoughts…

It certainly sounds like you have a complicated medical history/ situation, and that does make it really hard to decide what symptoms are caused by which condition, but it’s great that your hyoid bone surgery has helped with some things. The tingling you get in the front of your neck, was this there before the hyoid surgery?

With IJV compression, this can cause head pressure- especially after laying flat. Sleeping propped up can help, so worth trying to see if it makes any difference? And losing some hearing can be common with ES, pulsatile tinnitus (where you hear your heartbeat in your ear) is also common with IJV compression. Although often exercise can make the head pressure worse too, which it sounds like it doesn’t for you?

If the neck tingling wasn’t there before surgery then this could be a nerve healing?

There can also be some overlap between ES symptoms & TMJD ; have you looked into whether you could have that?

It will be interesting to see your scans, if your stylo-hyoid ligaments were detached but left in, if there’s calcification on them it could still be aggravating nerves in the area, or blood vessels… Have a look at the common symptoms section if you haven’t read that, it might help you identify some! @asm8ax - We prefer to see 3D dicom images as they’re easier for us to read & annotate. Images from the front, back & left/right sides are helpful. If your imaging was done w/o contrast, then your vascular tissues won’t be visible, only the bony tissues in your neck will be.

You can upload images from your computer using the underlined up-arrow in the menu at the top of your dialog box or you can drag & drop images from your computer desk top into the dialog box to share. We prefer not to receive complete dicom files because it takes a lot of time to sift through the images to find those that are relevant so posting only those that show the area from your hyoid, to your styloids/stylohyoid ligaments & up to the skull base are best. Please remove any personal information on the images before posting them.

So i generated these using the raw dicoms, i put the whole slice into the 3D slicer program I downloaded, then went to the “CT-Bone” view. the left side bothers me. As you can see, that segmented styloid is what I am talking about on left side of neck (though I realize it can be exaggerated). If there are any other “views” that would help let me know.

You definitely have IJV compression & it’s bilateral. Your left IJV is likely your dominant one as it’s substantially bigger than the right (though that doesn’t always determine dominance). I also suspect it’s dominant because of your left sided symptoms. Both appear to be compressed by your C1 vertebra w/ C2 possibly contributing on the left side.

You have abundant collateral veins bilaterally. These develop to help the IJVs when they aren’t draining blood & toxins from your brain at an adequate pace that keeps up w/ the rate the carotids are taking freshly oxygenated blood into your brain. The symptoms of IJV compression are caused by high pressure in your brain (intracranial hypertension) that results from the sluggish outflow of blood from your brain due to IJV compression.

Your right styloid is pretty dagger-like so I’m surprised you don’t have nerve symptoms on that side. The left styloid, though short, is thick & appears to have a “hook” on its tip which could be snagging on local nerves. It appears C2/C3 might be fused on the left side, however, that could be an anomalous imaging error. You also appear to have lost your lordotic cervical curve i.e your cervical spine is straight which can cause the upper cervical vertebrae to come into closer contact w/ the IJVs. The lordotic curve can be restored but it takes time & patience. Here’s a link to a post with good information about that:

Soo those images are before the hyoid surgery so just FYI lol. I fear my neck is cooked due to the C5 C6 fusion as far as the lordosis (I have tried pretty much everything). Dr. Matthew Pierce did the procedure though, and he had never done it before but he has done eagle so i told him to go at it. As far as I am aware there isn’t really fusing on c2 c3. That left segmented styloid I believe is what bugs me the most. The styloids are not elongated, but since my neck posture is cooked if I go to this new doc and he agrees to get the left segment out, I may as well do both sides I think… thoughts? And you’re sure of the IJV compression? My hyoid doc didn’t really mention anything to me about this, he seemed to think the styloids based on the CTA was fine, and the hyoid was the issue (which granted it did help).

Got it re: the lordotic curve. Sorry I read your initial post quickly & forgot about the C5-6 fusion. 🤪 Good to know C2-C3 aren’t fused & it’s just a visual artifact.

I am sure about your IJV compression especially based on the huge amount of collateral veins you have on the backside of your neck. Collaterals like that are NOT present where there is no IJV compression. Your styloids don’t appear to be contributing to the compression. It’s coming from the transverse processes of your C1 vertebra. It’s possible, your extensive collateral veins are compensating adequately for the lack of drainage through your IJVs so you aren’t terribly symptomatic from the vascular compression.

Your right styloid is elongated, albeit not hugely so, but it is longer than normal. Your left side isn’t, but that doesn’t mean it &/or the piece of calcified stylohyoid ligament aren’t causing symptoms.

Interesting that your images were pre-hyoid surgery as I wouldn’t have looked at your hyoid as a problem by the way it looks in the images you posted. Just further evidence that I’m not a doctor…😂

Yea I mean honestly in February when the image was taken, I really though symptoms were from that left segment of styloid, but apparently when I went into a “compromising” position in the CTA positional, the right great horn would contact the carotid and they thought that was the issue. But I fear there is more going on in my neck than just that. That is very i nteresting about the collaterals. Honestly I am pretty brain foggy most days especially after I eat… The styloids are under 2.5 mm or whatever the criteria was based on my measurements, but with my neck and how grossly hypermobile it is, I wouldn’t be surprised with that plus the fusion are making the styloids push into things they shouldn’t.

Brain fog is a typical symptom of IJV compression. Do you get headaches or have tinnitus or pulsatile tinnitus?

Tension headaches at the FRONT of my skull not the back which I hear is more typical. pulsatile tinnitus i would say yes, more on the left for sure.

Headaches anywhere in your head plus the tinnitus & brain fog = IJV compression symptoms. I’d say what I see going on w/ your IJVs in your imaging is affecting you, & it would be worthwhile getting a couple of consults from vES surgeons such as Dr. Costantino, Dr. Liu, or Dr. Cognetti (he’s booking next year in April now, I think).

Hmm I think I am just going to see this new doc in DC and see if he agrees theres potential vascular compression, otherwise maybe Pierce will just do it
