# Elongated styloids in CBCT

Hello I have been in quite pain, I got my cbct test done and it said i have enlarged styloids around 3.5 and 4cm in length, i am based in india and all doctors are shrugging it off that pain is not due to this because they are unaware. But i have seen a doctor for mostly everything for now and even gotten months of physiotherapy. I need support if i can share with files with anyone and they can see if this is causing the problem as i have immense brain fog also along with pain. Then i can decide what to do and which doctor to see as there are close to none doctors for this in india. Thanks.

I’ve sent you a welcome message with some info about the site… Feel free to post images & we’ll have a look, although we’re not doctors here… if you can make sure there’s no personal info on it though to protect your privacy. Although if your styloids are 3.5 & 4cms, they’re certainly long enough to cause symptoms! What pain do you have/ whereabouts?

I have immense head pain on the back of my head on the right side. I feel tired and unmotivated all the time. It’s difficult to decide this is what is causing my symptoms because doctors feel it’s unimportant to cause these issues. I am attaching one image for reference. Any help will always be helpful.

Being tired is common with ES. Head pain at the back of your head can be due to collateral veins forming- these are veins which can swell up to take blood flow from the brain if the Internal Jugular Veins are compressed by the styloids. This shows best on a CT with contrast, if you’re able to get a doctor to agree to send you for that? (It needs to be from the base of the skull down to the hyoid bone, and timed to show the veins in the neck).

Looking at your image, the styloid is longer than the ‘average’, it’s also pretty thick at the top, which is where the IJV and several cranial nerves exit the skull, so there can be compression commonly in this area. Also your styloid in this image looks very close to the C1 transverse process, which is another common point of compression- it’s not possible to tell for sure as sometimes looking at images from a different angle can show clearer, and of course because the veins aren’t shown in the CBCT… But looking at this I think it is worth getting a CT with contrast done as IJV compression could explain your symptoms.

@Hopee - Welcome to our forum! I agree with all that @Jules said. The symptoms you’ve mentioned are among those seen with internal jugular vein (IJV) compression & your styloid is very close to the TP of C1 as you can see from the very short red line I used to mark it. I have marked some other structures in your picture, too, so you can identify them more easily.

Parallel to your styloid is another bone that may be elongated which is growing from the roof of your mouth. If I am correct, it is the pterygoid hamulus bone. Is the roof of your mouth sore on one side? We’ve had a few members who’ve had an elongated pterygoid hamulus bone or bones (one on each side) that caused pain & swelling toward the back of the roof of their mouths.

I could get a ct test done. Also, do you suggest me to book a online consultation with any dr? So I could have the confidence this is what’s causing my problems. All this exploring and major nervousness has given my major trust issues after searching for a solution and I might want to keep a dr looped in to show my reports.

I’m unable to decide who to show as there are so many names i see.
