Hello, I am currently looking into getting a styloidectomy. I have had multiple non dye CT’s and a ct with dye, and a 3d CBCT I could have sworn that I would have had carotid compression on my scans based off of my stroke like symptoms that even have been sending me to the ER, but the docs say it’s not compressed by my styloid. I am worried when looking into surgery a doc will want a venogram as well and I do not react the best to the dye. On top of that I’ve had almost 5 CT’s within 3 months and I feel like that is too much especially with me being only 25. I was curious if there was another way than just the venogram? I recall hearing something about a special ultrasound for the similar thing at the Hauser neck center in Florida? I wasn’t sure if that would do the same thing. And just as a side question lately I’ve been getting very severe sudden panic attacks that will come and go in a snap of fingers it’s just a sudden onset feeling of terror as well a feeling of being in a dream like state and not fully here. I believe I have noticed certain ways I hold my neck to decrease and increase that feeling more (there is suspicion that the styloid is right up on the vagus nerve from what other docs have talk to me about). I do notice in the morning after waking up is when I usually feel best and the least in that “dream like” state, and as the day goes on it seems to gradually get worse as well as the increase in the sudden panic spells. I was just curious if any one else has that happen to them as well? And any help/advice/recommendations on the venogram situation? Thanks you so much.

@jakef8 - The symptoms you described in your post are very common with IJV compression but not so much with carotid compression. That includes feeling better in the morning when you first wake up & having symptoms increase as the day progresses. When the IJV is compressed, often between the styloid & C1 vertebra, the vagus nerve also gets compressed because it sits right next to the IJV within the carotid sheath in that area.

You can search for Dr. Hepworth’s ultrasound protocol on this forum using the magnifying glass icon & can give it to your referring doctor for ordering an ultrasound.

Have you tried converting any of your CT scans to 3D images so you can better see what’s going on in your neck? If not that would be helpful for you. You can post some of the relevant images here for us to take a look at. We often see things that radiologists & doctors miss or don’t mention that we consider important.

[radiantviewer.com](http://radiantviewer.com) is good for 3D conversion when using a PC & Bee Dicom Viewer App works best for Macs.

Just to add, that if you’re getting stroke like symptoms, do they happen when your head is in a certain position? Carotid compression can occur with head movements depending on the angle of the styloid, so if you have your head still during the CT this could be why it doesn’t show.

I agree that you’ve had quite alot of imaging already in a relatively short space of time, so I can see why you wouldn’t want any more doing!

Is your reaction to dye a mast cell or histamine related issue? For people who have histamine intolerance or mast cell issues or an allergy to dye, there are medical protocols that have been developed to try to reduce or prevent the negative symptoms someone may experience from dye.

I am not a doctor so I would not feel comfortable sharing what someone should or should not do, but organizations such as the Sloan Memorial Kettering Cancer center have more info on this online if your doctor is unfamiliar or if that’s a helpful avenue to pursue.
