@Mehran - Your styloids aren’t excessively long but they are very thick. It looks like you have space between your right styloid & the transverse process (TP) of C1 but on the left side it appears your styloid is sitting on the TP of C1 which means your internal jugular vein (IJV) & vagus nerve are likely being completely squashed. That could be the cause of the terrible symptoms you have.
Interestingly your C1 vertebra sits very close to your skull base so the surgery you need may be more challenging. You will need to have your styloid cut off above where the C1 vertebra is touching it & possibly also have part of the transverse process shaved off to make room for your IJV to open up & your vagus nerve to recover.
If there isn’t a surgeon in your country who can do the surgery you need, Dr. Aghayev in Turkey is very experienced with the surgery.

@ Mehran - I’m not sure what Dr. Aghayev charges for surgery. Without charge, you can get an an opinion as to whether or not he thinks you need surgery. You will need to send him your CT images to get that opinion. If he says surgery would help you then you can pay for a full initial consult with him & at the time he will go over your imaging in detail & discuss how the surgery would be done & what it will cost.
