I’m fairly certain those are you styloids based on the positioning. I compared yours with my own clearer imaging. I think you’re pretty justified to want a head/neck CT with contrast.
@Jete - If what @TML has marked are your styloid (it’s a definite possibility) then they are quite long, & it’s no wonder you have the symptoms you do. Perhaps you can take the image he marked to your GP & show him/her to make a better case for getting a CT scan w/ contrast.

The places you showed my styloids are, is the same place I found them when I was investigating other images and comparing. I even measured my styloid in program where my Xray is, it was 37.17mm long.
You just confirmed what I saw.
Now I’m feeling much closer to problem solving and don’t feel so bad thinking that it’s all just in my head. Thank you. ![]()
Radiologists measured mine at 3.3cm and ours look similar in length, so I’d say your measurements are pretty damn spot on! Glad we could help!

Did doctors removed your styloid?
I have a surgery consult but it isn’t until January. I have a few things going on. Both IJVs are being compressed between C1 and styloids, IJVs also being compressed by SCM muscles. And my hyoid bone is digging into my carotid arteries. So not sure what the surgeon has planned for me but hoping at the very least the styloids can be dealt with and maybe the hyoid.
Dizziness/vertigo can be a symptom of IJV or carotid artery compression. It will be good for you to get a CT with contrast so we can see where and what is being compressed for you! Your styloids are likely the main culprit, but there could be stuff happening in other spots too so it’ll be good to give your imaging a thorough look!

Hi, I’m reaching out to you today I BELIEVE you! It does not sound “all in your head”. Psychological stuff rarely presents with such a specific thing: ear pain- one ear! I started with similar 5 years ago, one ear (left), I won’t go into any other details, old stuff which is in my posts: but : ear pain has 2 causes, either it is coming from the ear or it is referred to the ear from structures nearby or nerves. Just because the drs gave not found the cause, it does not mean no cause exists. If you have instability in your neck, that needs to be looked into properly and if this is seen on xray, how can nothing been in MRI.
There is good and scientific information on here, great support and non doctors with more knowledge than some of the so called experts. Don’t give up, the answer is there but journey may be long. Be brave, be persistent ![]()

I have bunch of symptoms, not only pain in my ear. Pain radiates down to neck, I have constant feeling that something is stuck in my neck (but when I tell about this feeling doctors they are telling me that it’s psychological).
Mri doesn’t show instability. Positional xray in 2022. showed it.
sometimes I have pain (where tonsils should be), like fish bone is stuck and causing pain. But there is nothing wrong.
I live in pain for years, I used to it.
my main concern is about my vertigo. Yes I was having those vertigo episodes for 6 years, now and then. Once in a month or sometimes more or less. But now in 6 months time it is got from couple of days to weeks non stop. Most of the time I have to spend horizontally and not to move my head. At night I wake up from pain because I can not control my head position during my sleep.
I don’t even speak about brain fog and difficulty to think.
My gut tells me that there is something terribly wrong but no one knows what…
I am scared to death…
Keep searching for a doctor who will refer you for a CT scan with contrast, @Jete. Seeing what’s going on in your neck will make a HUGE difference in how doctors treat you & will open their eyes to the fact you’re not “faking it”. I will pray that you’ll find the doctor very soon who will refer you for the scan you need.

I don’t have this “fish bone” feeling 24/7. It’s just comes and goes. Maybe that’s why. I don’t know…
That is very typical of ES symptoms - they come and go. There are times when some symptoms come & stay, but most often they are intermittent which does make diagnosing ES challenging.
I’m sorry I can’t see anything in your MRI. All the soft tissues that are visible are covering up the bony structures that may be causing your problems.
