Between the skull-base and C1, your right styloid is compressing your right IJV. The left IJV is not compressed by any structures at this level. Note that your left IJV is your non-dominant (smaller) IJV.

At the level of C1, your right IJV is wedged/compressed between your right stlyoid and C1. Your left IJV is compressed solely by C1 (your left styloid is not contributing to compression).

At the level of C2, both of your ICAs make contact with your IJVs. Your ICAs are “tortuous” in that they go from an anterior (forward) position to a further back position. When your ICAs move backwards, they come in contact with the IJVs and there is some compression.

At the level of the hyoid, the right greater horn is in contact with the right CCA. There is no compression, but mechanical irritation can refer pain up and down the length of the left artery(ies).

Lastly, your right superior horn of the thyroid is in close proximity. I doubt this is causing issues, but I wanted to flag it.

Note that your CT involved you looking straight (the images I annotated) and with your head rotated left and right. The left and right head rotation positions had the same story as the standard straight position, so I did not include annotated images from those two positions.

I also noted that your left IJV is compressed well below your C1/styloid area & wonder what beside the ICA might be involved w/ that?
