Don’t know if you ever found a solution for your problems, but a CT scan of the neck from hyoid bone to skull base will confirm or deny ES. There are several doctors in CA who are quite experienced w/ ES & the surgery required to cure it. If you’re still reading posts from this forum, please reply & I’ll give you some references.
i dont get that when swallowing but does anyone get the feeling when tilting head to one side and sticking tongue out the hyoid gets caught on something and kind of pulls out finally? kind of like pulling a ball through a smaller hole and it finally releases feeling? hard to explain but did my best for an example.

That sounds a bit like some symptoms SewMomma experienced prior to her surgeries. Do you know if you have calcified stylohyoid ligaments?
I used to have trouble explaining my click too! You did a really good job! Ligaments were the smoking gun in my case.
SewMomma, did doctor explain why the ligaments cause clicking? I too have clicking on my right side. I have what appears to be calcified ligaments at the base of hyoid bone.
Hi! My calcification started at the bottom and went up too. I wish I could have had a microscopic camera so I could have seen exactly what caused the click, but my surgeon and I both believe the tension caused by the ligaments was making something “snag” when I swallowed. Either the hyoid was rubbing against another structure (throat area or spine possibly???) or the ligament itself was getting caught. Now 8 months after my second surgery - I still have the occasional tiny, tiny click-like sensation so I believe it’s the hyoid still settling down from being pulled. If I were at a “10” with clicking pre-surgery, it’s now barely at a “1”. It did drive me absolutely crazy though. So glad it’s gone.
Thank you SewMomma, i guess this is not very often , but my ligaments are calcified from bottom up as well, and similarly, I have a click. I’m so happy for you that your problems are gone ! By any chance, did you have TMD? I have my jaw pop on the right side, it started around the same time with the click in throat.
Yes you could hear my jaw pop with every chew back at that time. No more.
