Hello to all you clever people - I have tried hard but I still find the scans very hard to read. I had a styloidectomy, c1 resection and ijv decompression surgery six months ago. Now I have differing opinions from two neurosurgeons - the neurosurgeon who performed the surgery said there is very little C1 left to remove and the second neurosurgeon thinks I am still compressed by my c1. He has offered to do a revision surgery on the operated side plus surgery on my other side - I am nervous and uncertain what to do. Is anyone able to tell from my post op ctv if my ijv is still being compressed by my c1 or scar tissue? What do I need to upload? Thankyou to anyone that can help me. To be honest it’s very upsetting as I still have my horrible symptoms and my latest scan said my collateral veins had increased
I’m sorry that you’re still having symptoms, and that you have differing opinions from the VES docs…we can have a look for you, but obviously not doctors, & if they can’t agree ![]()
I’m not very good with tech, what format are the images on, I don’t know if some of these tutorials are helpful:
RadiANT DICOM Viewer tutorial - General - Living with Eagle
Or if you use a Mac computer, Bee Dicom Viewer app is best for Macs. It’s free.
Just make sure you remove any personal info…
@Gina1961 - I’m also very sorry your symptoms are still with you. It’s really great that the second neurosurgeon is willing to do a revision surgery & deal with C1 if it’s still the problem. There can also be soft tissues causing compression which are missed i.e. muscle, nerve, blood vessel, lymph node, fascia, scar tissue, etc. It should be obvious on your imaging if there are spots along your IJV that look flat which are below the C1 area as those flat areas can indicate other causes of compression which also will need to be dealt with for the best results.
Was the transverse process of your C1 vertebra shaved at all to make more space once the styloid was shortened? If not, even if the styloid is short enough to not be causing a problem now, it is possible for the TP of C1 to still be a problem if a) it wasn’t shaved at all or b) it was shaved but not enough was taken off.
It’s also true that in cases of bilateral styloid elongation & IJV compression, having both sides taken care of via surgery is often necessary to get good results.
Hello
My c1 was resectioned and neuro said only about 1mm left, second neuro played my ctv as video and it showed more c1 could be removed. It seemed like they had different operating techniques but first neuro says it will be a waste of time and has unsettled me. Scan report also said my collaterals had increased on that (operated) side so something needs to be done. I’m just nervous to have another operation and for it not to work. I was hoping I could upload the video somewhere and someone could look at it for me and give their opinion ![]()
Thankyou
I have it in a WhatsApp video - it’s about 4 minutes long - that’s how they sent it to me ![]()
I’ll send you a private message…
Thankyou - I’m off to bed now - will send tomorrow
Sounds similar to me. I need a revision of C1as two of the well known providers told me my before and after imaging is still the same. I am still very compressed. Looking to start this nightmare all over again.
It’s awful isn’t it - who have you consulted with? Did you have styloidectomy, c1 resection and ijv decompression? I’m hoping to have further surgery with Dr Aghayev - just a little overwhelming as it’s so far from home and my original surgeon said there’s minimal left to remove. But I’ve spent the afternoon consulting with Chatgpt and Gemini .. Gemini can read videos and Chatgpt can read screenshots - Gemini identified the timestamp on the video where the c1 lip was compressing my ijv which I then uploaded into Chatgpt - they both agree with Dr Aghayev - very interesting … so although there is only 1mm left it is still a problem and has to go - I’m going to get my other side done at the same time - I’m not looking forward to it at all as I have such issues sleeping but I guess it will be worth it in the long run - at least I hope so - good luck to you ![]()
I’m very sorry that both you of are having to face round 2 of surgeries that weren’t done as completely as necessary the first time around. I’ve sort of been there myself so can sympathize. I wish it wasn’t the case, & hopefully,
, the second time around provides the symptoms relief you expected from surgery #1.
Please let us know when you have your surgery dates.
thankyou! I’ll be booking as soon as I have the $'s ![]()
Hi we’ve booked - surgery is 23 June ![]()
I’ve put your surgery date on my calendar, @Gina1961! I will be praying for you that day especially. I’m glad it’s not too long from now!
thankyou
you have been so amazing with your constant support since April 2025 when I discovered I had this condition - it really is wonderful to have you and Jules ![]()
