This image looks like it is more the C1 process causing compression, rather than the styloid, so you may need the C1 shave as we’ve said, I would suggest you try & get a referral with Dr Costantino as we’ve suggested…
@6tdog6 all that compression of the left IJV is due to the C1 transverse process. The styloid isn’t touching the IJV at all. That is why the axial view is better to assess the pinch. In these photos you can see that your left IJV (right side of image) is pancaked against C1 and the styloid is well distanced away.
That’s not to say your styloids aren’t causing any nerve issues, but I think your C1 is the IJV culprit.

I might opt for a surgeon we have on our list who does both styloidectomies and C1 shaves. They might find that your styloids are causing issues, and are also equipped to deal with C1. I think @Isaiah_40_31 @Jules could be best to help. I’m not very knowledgeable about the doctors list unfortunately!
It would be for an ENT & neurosurgeon as @TML noted. Dr. Costantino who is in your state is one of the most experienced surgeons we know of for the surgery you need. It would be worthwhile having a consult with him.

Persistence is key, @6tdog6. You can also try calling at different times of the day. Some offices have less calls coming in 30 min before they close for lunch or later in the afternoon like 3 pm on. First thing in the morning is usually the busiest phone call time for doctors’ offices especially on Mondays.
Tysm, I’m pretty new to this condition. Just wanted to ask if you think it could cause such debilitating fatigue? I do have some pain but it’s not even close to being the number one issue for me
@6tdog6 - Debilitating fatigue is definitely a possible symptom of ES. It can come from nerve or vascular compression. Many of our members have noted fatigue as among their most annoying symptoms.
