Questions about carotid sheath

Hello all, I had a few questions about the carotid sheath that are probably going to seem a bit ridiculous - I’m still learning, so apologies in advance!

From my understanding, ES/VES can be caused by the styloid process or a hardened stylohyoid ligament pressing down on structures within the neck. This can include the jugular vein, carodid artery and the vagus nerve.

I’ve read that the carotid sheath contains, amongst other things, the jugular vein, carotid artery and vagus nerve. I would imagine this means that they are therefore quite close to each other.

So - if those structures are all within the carotid sheath, and the carotid sheath gets compressed by the styloid process, does this mean all of the structures within it are compressed too?

(I don’t see how just the jugular could be compressed and not the carotid, if they are both contained within the same fibrous tunnel)

And following on from this, does surgery therefore free up the complete carotid sheath rather than individual aspects within it (e.g., just the jugular vein)?

I imagine I am wrong but would appreciate anyone telling me why! :stuck_out_tongue_closed_eyes:

As far as I’m aware, the carotid sheath is a term for the fascia (connective tissue) which surrounds the carotid artery, IJV & vagus nerve , so you’re right about that, but as those blood vessels are separate from each other within the connective tissue, they can be compressed separately by the styloid, depending on the angle it grows. It’s a fair question , as I must admit I didn’t realise that they were all contained within connective tissue together! And it explains how the vagus nerve is very often compressed & why we see so many members with vagus nerve symptoms…
The IJV leaves the base of the skull in a small hole called the jugular foramen, right beside the styloid process, the internal carotid artery leaves the skull from the carotid canal, right beside each other. So a very thick styloid can compress the IJV right from the skull base…this site has an image of the skull base which is quite helpful:
