Doctor said its not eagle, but can you check again

Your styloids do look a bit longer than the average, but your C1 processes look quite long too, so I don’t know if they’re causing any issues?
In this imaging your hyoid looks like the greater processes are really long, and the right side especially curved with a hook on the end- it might be that the stylo-hyoid ligament has calcified that end upwards…
The thyroid cartilage looks pretty chunky as well as we’ve noted before, so that is worth asking about…

@mjzago - I’m sorry that only an angiogram was done & not a venogram as your IJVs are not visible in the images & their condition/positions also needs to be considered. I agree w/ @Jules that your C1 vertebra looks extra wide so could be impacting your IJVs even though your styloids may not be involved though the right styloid is long enough it could be, too.

For the most part your carotids look ok, with the exception of the left ECA/carotid bifurcation which could be getting poked by the left greater horn of your hyoid. The curve in the right greater horn looks like ti could actually be beneficial as it appears to be what’s keeping the right greater horn from making contact w/ your right ECA. The right greater horn could be tangling w/ nerves, though.

I also agree w/ Jules that your thyroid cartilage looks excessively calcified which in tandem w/ the greater horns of your hyoid could be causing Hyoid Bone Syndrome or Clicking Larynx Syndrome. Definitely worth talking about with the doctors you’re consulting.

Hi everyone,

First of all, I’m sorry for taking so long to write again. The last few months have been extremely difficult for me, and I had to make several hard decisions along the way.

I was hospitalized in a psychosomatic clinic in Hanover for 8 weeks. Unfortunately, my pain is still very strong. At the moment, I am taking Tegretol and Pipamperone.

Since my last update, I have done another CT angiography with 3D reconstruction of the neck and head. I also managed to consult with Dr. Blake in Hamburg and Dr. Martini. Dr. Martini told me that my case is not classic Eagle Syndrome, but that I might be in a “gray zone,” and he offered surgery as a possible option. The same opinion was given by Dr. Oliveira in Brazil during my second teleconsultation with him.

When I consulted Dr. Black in Hamburg, he told me that my case was not Eagle Syndrome. Because my pain worsened significantly, I decided to return to my home country, Brazil, to be close to my family. It seems that in Germany there are very few—or possibly no—specialists who work specifically with thyroid cartilage or the hyoid bone.

After arriving in SĂŁo Paulo, I was able to consult with an ENT specialist, Dr. Pascoal, who performed a laryngoscopy. I will post some of the images here as well. Dr. Pascoal identified problems in the right thyroid cartilage and also on the left side. He said I have no signs for eagle syndrome, and my problems lays on the greater cornu of the thyroid cartilage. So I underwent an infiltration with steroid and corticoid in the right greater horn / right side of the thyroid cartilage. The infiltration brought some improvement, but I am still very debilitated.

I really don’t know what is the yellow mark I highlighted here. Is my greater horn of the thyroid cartilage?

I will wait a few more days to evaluate how things evolve and then decide on the next steps together with the doctors who are currently treating me here in Brazil.

I can post more images if that would be helpful. I would also be very happy to know if there are any doctors in Brazil or Germany who treat thyroid cartilage or hyoid bone conditions.

This forum has been incredibly useful to me. I am receiving a lot of help, support, and understanding from many members, and I’m very grateful to this entire community. Thank you all so much, and I wish everyone a very good end of the year—hopefully with less pain and more relief for all of us.

Just to clarify my experience with the doctors in Germany

Dr. Blake is a very kind and pleasant person, and I respect him a lot. He has clearly done many Eagle Syndrome surgeries and is very experienced. He told me directly that I do not have Eagle Syndrome. However, when I asked specifically about the hyoid bone and thyroid cartilage, he was not able to give me clear answers or direction. He also speaks a little Portuguese and even mentioned that he enjoys Brazilian music and dancing.

With Dr. Martini, I only communicated by phone, and it took some time to receive replies. He seemed very focused and experienced as well. He also said my case was not classic Eagle Syndrome, possibly a grey-zone case, but likewise could not give me clear guidance regarding the hyoid bone or thyroid cartilage.

Thank you for your update, @mjzago. I’m so very sorry that your symptoms are very severe. I hope that the work Dr. Pascoal is doing will make a big difference as your body begins to benefit more from the steroid injection. If that brings more relief, is there a surgical procedure he would be able to do to remove any excessively calcified portion of your thyroid cartilage or even on your hyoid bone? I gather, he also knew of no one he could refer you to for more invasive care i.e. surgery if you need your hyoid bone resected.

I’m glad you’ve received good support from our members & hope that keeps up at least until you get the answers & treatment you need to start feeling better.

If the steroid injection that Dr Pascoal did seems to help, then maybe that’s a good indicator that he’s right about where your issue is? We don’t know about any doctors doing thyroid cartilage reduction in Europe, & the same with HBS, not to say it’s not being done, but certainly nobody has any good recommendations.
@Calista21269 is in France, & is looking into HBS or ES, and trying to find a surgeon in Europe but is trying US doctors, here’s her discussion:
