Avec l’expérience que j’ai acquise avec mes deux fils, qui ont tous les deux subi une décompression chirurgicale bilatérale des veines jugulaires internes, le Dr Aghayev en est maintenant à un stade où il a tellement peaufiné sa procédure chirurgicale pour la décompression totale de la veine jugulaire que le temps de convalescence est rapide.
Avec mon fils qui a été opéré en juillet 2026 (il y a bientôt deux mois), il a été capable de bouger la tête 24 heures après l’opération et de dormir sur le côté.

Drawing from my experience with my two sons, both of whom underwent bilateral surgical decompression of their internal jugular veins, Dr. Aghayev has now reached a stage where he has refined his surgical procedure for total internal jugular vein decompression to the point that recovery time is rapid.
With my son who was operated on in July 2026 (nearly two months ago), he was able to move his head and sleep on his side within 24 hours of the surgery.

Hi Jordan, In my opinion Dr Aghayev is a wise choice given these axial views of your right and left internal jugular veins. It looks like you need bilateral C1 transverse process partial resections at the least.
You have amazingly well developed transverse C1 processes that are very wide with a large lip at the end that are hindering both the right dominant internal jugular vein (left of your screen) and your left non-dominant internal jugular (right on your screen). You have a severe stenosis of your left non dominant internal jugular vein at the level of C1 which certainly doesn’t help the situation at all.

I usually release the stylomandibular ligament along with the styloid when I do the initial surgery. It is unclear if this was released in your initial surgery, I suspect it was. I do not see significant jugular vein comp0ression or C1 issues on your scans.
I think Dr Hackman only does styloidectomies he doesn’t do C1 partial resections or internal jugular decompression surgery. I’ll let @Isaiah_40_31 confirm this or not.
Given the conflicting answer from Dr Hackman I would definitely get a cerebral CT scan with contrast to check the veins in your brain to remove any doubts you might have either way. All I can say is his response doesn’t explain the presence of collateral on the left side of your 3D rendering.
Also from what I can see both your right and left internal jugular veins are flattened by the presence of your prominent C1 transverse processes at the C1 level.

@Emerald is correct that Dr. Hackman only does styloidectomies, albeit up very close to the base of the skull. If C1 shaving is necessary to fully allow the IJVs to open, he does not do that. We have had a few members w/ IJV compression who’ve had to have revision surgery after having initial styloidectomies done by Dr. Hackman due to their need for a C1 shave. However, the surgery he does has provided decompression for IJVs that are predominantly compressed by the styloids in many cases.
It’s possible a cerebral CT scan or a catheter venogram with manometry (checking pressure gradients w/in the veins in the areas above & below the compressions) would be a good next step.

Hi Jordan, I understand your concern given the number of operations that you already have had and can also understand your trepidation about going to another country for treatment. You have 2 “options” ( I don’t know whether “option” is the right word given the stance taken by doctors in France 
.Let’s say “possibilities”).
FIRST OPTION
- You go back to your family doctor armed
with this recent ( May 2026) publication on facial pain and internal jugular compression being the cause due to petrosal sinus congestion. Internal Jugular Vein Entrapment: An Underrecognized Cause of Facial Pain Also print out Figure 1. and give this image to him first before giving him the publication.
Explain that petrosal venous congestion needs to be excluded and this is the reason why you need a cerebral CT scan with contrast to show your cerebral veins and in particular the venous petrosal sinuses next to your trigeminal ganglion.
If your doctor doesn’t believe you have internal jugular vein compression on the right or a severe stenosis of the left internal jugular vein then you will require a second opinion of the CT scan that you already have of your neck. This will be a more difficult and delicate step. You could try getting in contact with with Dr Stephanie Lenck a neuroradiologist at Pitié Salpetrière Paris. She was recommended by Dr Hui, a prominent neuroradiologist in the field of cerebral venous congestion in the US to another member on this forum @CristianSerious. Maybe she has a facebook page or something. You could try asking the neuroradiology departement if you could have her email address because you have a scan that you would like her second opinion on .
You could also use the service of floy.com a German second opinion service on internet. Explain to them you would like a second opinion on a CT scan with contrast as you suspect internal jugular vein compression on your right, a severe stenosis of the left internal jugular vein and overdevelopment of collateral veins at the C1 level. Tell them you would need a radiological report and images detailing these anomalies on the axial and sagittal views and pictures of 3D rendering showing these 3 anomalies. Say you require these images to show your family doctor. This service will cost you about 200 euros.
I would not use the service of deuxièmeavis.com . They only know about internal jugular vein thrombosis associated with having the face of a michelin man.
SECOND OPTION
- I know Dr Aghayev has already sent you a video explaining you CT scan with contrast. But maybe you should explain the situation to Mr Bassam, the medical assistant (who is a doctor) via Whatsapp. Explain that you are not sure about making the right choice, explain that you would like a CT scan of your cerebral veins to see what is going on with your petrosal sinuses. It would be easy enough for them to organise this scan very quickly if you decide to go to Istanbul. Send Mr Bassam the May 2026 publication. I’m sure they know about it anyway but it wouldn’t hurt to send it to him. He won’t be offended. You could also organise a consultation with Dr Aghayev online to talk to him directly about your concerns.

Bonjour emerald , le docteur aghayev me fera une imagerie là-bas si je veux être rassuré , je lui ai montré la réponse du docteur hackmann et m’a tout simplement répondu qu’il ne souhaitait pas voir les réponse de ces chirurgiens qu’il y avait une compression évidente de la veine jugulaire gauche et qu’il ne comprenait pas que eux ne la voit tellement elle était évidente . Merci beaucoup pour votre temps vous avez tous été d’un soutien énorme . Je reviendrais vers vous pour vous raconter le périple qui m’attend mais je vais me battre pour mon fils . Merci encore

‘Hello Emerald, Dr. Aghayev will do an imaging over there if I want to be reassured. I showed him Dr. Hackmann’s response, and he simply told me that he didn’t want to see the responses from these surgeons, that there was an obvious compression of the left jugular vein, and that he couldn’t understand how they didn’t see it, it was so obvious.
