What do you think this tissue is?

blue is two ijv
and The area is directly above c1 transverse process

Not an expert, I would guess soft tissue like muscle? I can’t open your other images, so not too sure I’m afraid. Your styloid in the image above looks pretty thick, so could well be contributing to IJV compression.

I agree w/ @Jules that the area you circled in black are muscles. There are muscles that can add to IJV compression such as the SCM & posterior digastric. I’m not sure if either of the muscles you have circled are one of those as I’m not that good at reading CT axial images. You do have the IJV & styloid circled correctly.

Have you decided where you’ll go for surgery yet or do you have surgery scheduled?

@NoneElongatedStyloid - Do you know what vertebra level the slice w/ the red spot is i.e. C1, C2, C3, etc.?

We’re most familiar with what we see at the level of C1. Below that, we don’t have experience in identifying structures in the axial images. I’m also not familiar w/ the particular area of the coronal slice you put up.
