Dehiscence of the sigmoid sinus and dehiscence of the carotid artery within the middle ear

Hello All,

I had my IJV decompressed in December 2025. However, I had recent high resolution CT scans on May 12th, 2026 and they revealed that I have:

  1. Dehiscence of the sigmoid sinus
  2. Dehiscence of the carotid artery within the middle ear

I know that his group is for ES/VES but would anyone know about those conditions?

It’s not something I know anything about, it’s been mentioned occasionally, & @Isaiah_40_31 has been diagnosed with dehiscence of the sigmoid sinus I think…
@Chrickychricky has been diagnosed with carotid cochlear dehiscence, not sure if that’s the same as you have been diagnosed with, but sounds like it could be, here’s her post about it:

I’m sorry I can’t give you any more info, did Mr Axon suggest if there is treatment/ surgery for this?

@Hamela I have carotid cochlear dehiscence which is what I assume you mean by carotid dehiscence of the middle ear. It means that that bone that usually encases the carotid as it passes by the middle ear has a hole or opening in it allowing the sound of the blood flowing through the carotid to be transmitted to the inner ear. That is the assumed root of my pulsatile tinnitus. Surgical intervention is too dangerous with high risk for stroke or deafness.

@Hamela,

I’m so sorry that you have dehiscence of any sort. It’s known that high pressure in the brain from IJV compression can cause erosion of the tiny/thin bones in the inner ear. Usually we hear about Superior Semicircular Canal Dehiscence resulting, but it seems there are other areas that can suffer as well based on @Chrickychricky 's & your reports.

My dehiscence was mechanically caused i.e. the result of a surgery to place a stent in the endolymphatic duct in my inner ear to try to stop progressive hearing loss. It’s a tight space to operate & a bit too much bone was removed so I can now hear the blood flow sounds in my left ear which manifest as a constant loud hum. I can say I sympathize with what you’re dealing with, but I know that doesn’t fix anything. :hugs:

Yes, he suggested to fix the sigmoid sinus dehiscence and he is very confident in the results. He will also remove the fluids I have in my mastoid air cells. However, he said he never operated on a dehiscent carotid artery and he does not want to do that piece yet.

I’m glad you’re able to get the sigmoid dehiscence taken care of & fluid removed from your mastoid air cells. That should help significantly. Please let us know when that surgery will be done. Who is doing your surgery?

oups…forgot to write that my carotid artery dehiscence is unrelated to the cochlea. It is only related to the middle ear which can be operated but I need to find a specialist interested in that area.
