More experienced members will chime in but it certainly looks like a very sharp styloid on the left side of the image.

@Mehran I asked ChatGPT to look at this panoramic dental X-ray and it noted long, thin radiopaque structures on both sides in the upper neck area behind/near the mandibular rami, consistent with elongated styloid processes or calcified stylohyoid ligament segments.
I know CT/CTA with 3D reconstruction is much better for confirming styloid length, skull-base origin, angle, and relationship to C1/carotid/IJV, but I wanted to share this because the styloids do appear visible on the pano.
@Mehran - I’ve annotated your image. I can see a pretty significantly elongated on the left side of the pano x-ray image you posted (that’s the right side of your neck) & just the tip of your left styloid (right side of the picture). The difference in how much I can see of each may be related to their lengths being different &/or the angles they’re growing being different. The fact that they are visible in the picture you posted indicates they are longer than normal & there’s a good likelihood they’re causing at least some of the symptoms you have.
I agree that the styloids are visible both sides, and certainly worth trying to get a CT done from the skull base to the hyoid to see them better!
